Paid Time Off (PTO) Policy 

Effective Date: January 1, 2024 

1. Purpose: 

The Paid Time Off (PTO) policy at Unique Inventive Strategies for Home Care is designed to provide employees with time away from work for rest, relaxation, personal activities, and to attend to personal or family needs while ensuring operational continuity. 

2. Eligibility: 

All full-time employees are eligible for PTO benefits. Part-time employees may be eligible for pro-rated PTO benefits based on their regular work schedule. 

3. Lay Staff Accrual

PTO will accrue based on an employee’s length of service as follows: 

• 1-3 years of service: 3 days per year 

• 3-5 years of service: 4 days per year 

5+ years of service: 5 days per year

10. Administration: 

The HR department is responsible for administering the PTO policy, including tracking accruals, monitoring usage, and ensuring compliance with company guidelines and state regulations. 

11. Policy Review: 

This PTO policy will be reviewed annually and may be revised as needed to ensure alignment with company objectives, industry best practices, and legal requirements. 

12. At-Will Employment: 

This policy does not constitute a contract of employment and is subject to change at the discretion of Unique Inventive Strategies for Home Care. Employment with the company is at-will and may be terminated by either the employee or the company at any time, with or without cause or notice. 

13. Medicaid:

All full-time employees are eligible for Medicaid benefits. This includes the Pediatric and Adolescent In-home and Community Services program. The Medicaid benefit provided by Unique Inventive Strategies for Home Care covers children with disabilities and psyche diagnosis such as ADHD, ADD, depression, autism, bipolar disorder, trauma, and others.

Please submit this form to your supervisor and HR representative for approval at least two weeks prior to the start of your requested leave.
